Add your Agency and Programs to the 2-1-1 Santa Clara County Database

The 2-1-1 System connects people through a database designed for providing human service information.
To be considered for inclusion in our database, please follow these steps.

1. Verify your agency and programs fit our Inclusion Policy.
2. If your agency qualifies, submit the following forms below. Please read the instructions carefully. Multiple forms are required.
3. Once the data has been entered into the database, the designated 2-1-1 contact will be emailed with a copy of the agency record.
4. Information provided in their entirety will be prioritized first.

  • Agency Profile - Please fill out this form for your administrative office or headquarters.
  • Site Profile - Please fill out one form PER site or location within your organization offering a service. Multiple forms may be required depending on the number of sites at your agency. You do not need to fill out this form if your headquarters is the only site within your organization.
  • Service Profile - Please fill out one form PER service offered in your organization. Multiple forms may be required.

Please complete these files in their entirety to be considered for updates to our database. Fully completed agency, site and service profiles are given priority for data entry and updates into the database.
